Delivery of Service Through the Wellness Approach
Home and Community Care - HACC has experienced a significant demand for home based services. The implementation of the Wellness Model of service delivery will allow the available resources and funding targeted at those clients most in need.
On essence, the Wellness Model maximises client's potential by supporting a client to gradually be capable of doing more for themselves so they can achieve greater independence and improved health.
This is accomplished through goal setting with the client following an assessment.
Support workers assists in achieving those goals over time and gradually assigning more responsibility to the client as physical abilities improve, and functional independence is gained.
We must all remember that remaining sedentary is hazardous, and can ultimately cause loss of function.
Bear in mind the old adage "if you don't use it, you lose it".
