How to Access Services from TAPSS
Individuals can refer directly to TAPSS and a staff member will make an
appointment to come to your home to do an assessment.
Referrals can be made by a family member, yourself, carer, GP, hospital or
other health service.
Eligibility for service is clearly defined by an objective assessment process to
determine whether a client is eligible for Home and Community Care funding.
All our services are offered based on a Wellness Approach.
When you are offered a service an agreed appointment time of delivery of
service is negotiated with you.
Ongoing service is delivered at this appointment time, however in exceptional
circumstances (eg. staff illness), service may not be able to be delivered at
this appointed time.
If this does occur you will be contacted and an alternative arrangement will
be agreed.

Review
Care needs will be reviewed and assessed according to need. A reassessment
will be conducted annually which is a requirement of the HACC program.

INFORMATION SHARING
All information relayed to TAPSS is strictly confidential.
De-identified demographic and service provision information is routinely
provided to the HACC Program for planning purposes.
